How is the Care Coordination Center different from home health?

The Care Coordination Center home program is an primary care extension. This means that the care provided by your primary health care provider is extended into the home setting. There is never a “discharge date” for the patients on the home program. As long as you feel that you need the services, you are kept as a patient. This means that patients who require outpatient physical therapy services, those needing additional supervision by a nurse, or those with chronic illnesses, all qualify for our program.


Does my insurance cover the services provided by the Care Coordination Center?

We are currently in the process of joining the network of several private insurance companies, as well as Medicaid. We are accepting Medicare but all others will be reviewed on a case-by-case basis at this time.
